Overview
As men get older, some begin to notice subtle changes in their penis. It may appear smaller when flaccid, feel less full than it once did, or no longer achieve the same firmness and size during an erection. These changes can happen gradually, often becoming more noticeable in a man's 40s, 50s and beyond.
But does the penis actually shrink with age? The answer is that it can, although not every man experiences measurable tissue loss. Changes in circulation, erectile function, body composition and connective tissue can all influence how the penis looks and feels. At Aiva, we believe understanding these changes is the first step towards choosing the right treatment.
Why Does the Penis Lose Size With Age?
Ageing affects tissues throughout the body. Facial volume changes, muscle mass gradually declines, and skin loses some of its elasticity. The penis is not entirely exempt from these biological processes. However, penile ageing is more complex than simply losing collagen or tissue volume. Two factors are particularly important.
1. Changes in Tissue Structure
The penis contains smooth muscle, collagen, elastic fibres and specialised erectile tissue. With ageing, particularly in men with longstanding erectile dysfunction or vascular disease, changes in smooth muscle and connective tissue may affect tissue elasticity and erectile function.
In some circumstances, fibrosis or scarring can contribute to genuine shortening or changes in penile dimensions. However, significant structural shrinkage is not an inevitable consequence of normal ageing.
2. Reduced Blood Flow and Erectile Firmness
A healthy erection depends on efficient blood flow and the ability of the erectile chambers to expand. As vascular function declines, erections may become less firm. This can make the penis appear shorter or narrower, even when its underlying anatomical dimensions have not significantly changed. Reduced erectile quality is therefore sometimes mistaken for permanent penile shrinkage.
Weight gain can also conceal part of the penile shaft beneath the surrounding pubic fat, making the visible length appear shorter.
Can Penis Filler Restore Lost Volume?
For men concerned about reduced penile girth or a less substantial flaccid appearance, non-surgical penis enlargement may offer an option. At AIVA, our Hybrid-Ultimate® penis filler treatment is designed to increase penile circumference by adding volume within the superficial tissues surrounding the shaft.
Unlike treatments targeting erectile function, penis filler addresses external dimensions. The treatment can produce an immediate increase in girth and a fuller flaccid length and appearance, with results varying according to anatomy, treatment volume and individual response.
Hybrid-Ultimate combines hyaluronic acid with a biostimulatory component intended to encourage collagen formation, and biostimulatory materials have demonstrated collagen-stimulating properties in other aesthetic applications. However, penis filler does not reverse ageing within the erectile chambers or restore erectile function.
The Role of Daily Tadalafil in Penile Health
While filler addresses external volume, tadalafil targets a different aspect of penile health: erectile blood flow. Tadalafil is a prescription medication that works by inhibiting phosphodiesterase type 5 (PDE5), helping improve blood flow during sexual stimulation. For suitable men, daily doses of 2.5 mg or 5 mg can improve erectile function without requiring medication immediately before intercourse.
Research has also explored potential benefits involving endothelial function, penile oxygenation and tissue changes associated with erectile dysfunction. However, evidence that daily tadalafil prevents normal age-related penile shrinkage, regenerates erectile tissue or permanently reverses fibrosis remains limited. Tadalafil is unsuitable for some patients, particularly those taking nitrates, and requires an appropriate medical assessment.
Aiva Approach to Penile Rejuvenation
At Aiva, we distinguish between two concerns that are often confused: penile size and erectile quality. Hybrid-Ultimate penis filler may improve external girth and flaccid appearance, while appropriately prescribed tadalafil may improve erectile function. For selected patients, these treatments can address different aspects of penile appearance and performance.
Neither treatment should be considered a universal solution to ageing. Our philosophy is simple: understand what has changed, identify the underlying cause, and recommend treatment only where there is a clear clinical benefit.
